Behavioral Health Case Manager
Provides case management services that help Midtown Health Center patients gain access to needed medical, behavioral health, and other services. Position involves assessment of the patient to determine needs; referral and related activities to help the patient obtain needed services; and monitoring, follow-up, and evaluation activities. The position provides patient education and support as appropriate. The goal of the case manager is to work with patients as referred/assigned to gain self-sufficiency and maximized health through self-management.

Full or Part-time Dentist
Midtown Health Center is a Federally Qualified Health Center (FQHC) located in downtown Norfolk, NE. The clinic is expanding services to meet current need by adding an additional Dentist. This position can be Full-Time or Part-Time. Midtown offers a full range of medical, dental, and behavioral health services in an integrated care model. Midtown has a new main facility, which opened in 2014, with an established satellite site in Madison, Nebraska, a second satellite site in Norfolk that opened in 2016, and most recently, a mobile dental unit which opened in 2017. Medical Clinic hours are 8:00 a.m. – 6:00 p.m. Monday through Thursday and 8:00 a.m. – 5:00 p.m. on Fridays; we are not open on weekends at this time. Dental Clinic hours are 7:00 a.m. – 6:00 p.m Monday through Friday. Currently, the clinic consists of 3 full-time Physicians (two family practice and one pediatrician), 4 APRNs (three family practice and two women’s health), 1 physician assistant, 2 full-time Dentists, 2 Dental Hygienists, and 1 full-time Licensed Mental Health Practitioner. Our team is passionate about providing great care to all patients—uninsured, underinsured, and insured!

Midtown Health Center has a full-time position open for a bilingual front desk receptionist. The ideal candidate would have a minimum of 2 years of experience working in a fast pace office environment and have excellent computer skills, outstanding communication skills and background in providing great customer service, but willing to train the right individual. Applicant must have a valid Nebraska driver’s license with clean driving record.
