We are a non-profit 501(c3) Federally Qualified Health Care Clinic (FQHC) serving the greater Norfolk and Madison area.

Midtown Health Center receives HHS funding and has Federal Public Service deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Norfolk Clinic Medical Hours:
302 W. Phillip Ave.

Medical Hours: Monday-Thursday 8:00am-6:00pm
Friday 8:00am-5:00pm

Dental Hours:
Monday – Friday 7:00am-6:00pm

Madison Clinic Medical Hours:
222 Main St.

Monday-Thursday 8:00am-6:00pm
Friday 8:00am-5:00pm

West Point Medical Hours:
435 Monitor St.

Tuesdays 8 am-5 pm
