Experience a Higher Quality of Care
To provide high quality medical, behavioral health, and dental services that are affordable, accessible, and patient centered in an atmosphere of trust and respect.
Midtown Health Center (formerly Norfolk Community Health Care Clinic) was established in October 1999 in Norfolk, NE. Since then, MHC has expanded into the Madison and West Point communities, adding more health services and providers along the way. We are a non-profit 501 c3 Federally Qualified Health Care Clinic (FQHC) serving the Northeast Nebraska area.
The board of directors consists of 10 volunteers from our local communities with expertise in various areas. They govern the clinic’s operations and seek funding to provide high-quality, affordable health care to residents of Norfolk, Madison, West Point, and surrounding areas.
Currently our four clinics employ a staff of 95+ between the medical, dental, behavioral health and specialty services.
You’re not just another patient to us. As a Patient-Centered Medical Home (PCMH) we truly care about you and your family. We have 24/7 access to care with our after hours line. All our providers communicate clearly to make sure you understand you condition(s) and all of your options. As a PCMH clinic we also will help get you information about other services to best fit your needs and keep you healthy. Your Care Matters Most.
