About Us

Health Center (formerly Norfolk Community Health Care Clinic) was established in October 1999 in Norfolk, NE. Since then, MHC has expanded into the Madison and West Point communities, adding more health services and providers along the way. We are a non-profit 501 c3 Federally Qualified Health Care Clinic (FQHC) serving the Northeast Nebraska area.

The board of directors consists of 10 volunteers and consumers from our local communities with expertise in various areas. They govern the clinic’s operations and seek funding to provide high-quality, affordable health care to residents of Norfolk, Madison, West Point, and surrounding areas.

Currently our four clinics employ a staff of 100+ between the medical, dental, behavioral health and specialty services.

You’re not just another patient to us. As a Patient-Centered Medical Home (PCMH) we truly care about you and your family. We have 24/7 access to care with our after-hours line. All our providers communicate clearly to make sure you understand you condition(s) and all of your options. As a PCMH clinic we also will help get you information about other services to best fit your needs and keep you healthy. Your Care Matters Most.

1999: Established

Midtown began as the Norfolk Community Health Care Clinic (NCHCC) to provide low-cost health care to those without insurance. The clinic was open two hours per week, located in a local church basement.

2008: FQHC Funding

After several years of relocating to remain operational, the clinic became recognized as a Federally Qualified Health Center (FQHC). FQHC funding allows the clinic to focus health care on those with additional barriers to care.

Our Madison clinic came to be, which co-located with Madison Chiropractic.

2009-2010: Dental & Behavioral Health Added

Oral Health was added to our list of services offered.

Behavioral Health Services were initiated.

2014: Name Changed

The Norfolk clinic relocated to Phillip Avenue and officially changed its name to Midtown Health Center

2016: Expansion

Dental services were expanded to Madison and Midtown opened an additional clinic location on Norfolk Avenue.

2017-2018: Mobile Dental & School Therapy

Midtown expanded dental services by adding a mobile dental unit. Through a letter of support, the Northeast District of the Nebraska Dental Association strongly endorsed Midtown’s ownership and operation of the mobile dental unit.

Added school based behavioral health services for students in Norfolk Public Schools.

2020 & 2023: Remodel/Expansion

Midtown Xpress Walk-In clinic opened at 210 S. 3rd St. in Norfolk

A clinic location in West Point was added at 303 Plaza Dr.

Midtown’s Phillip Ave. location underwent an extensive remodel to the waiting room, pediatric, and dental areas. Midtown also expanded services at Northeast Community College and Behavioral Health Specialists

Future: Expansion

Midtown purchased the building located at 423 Madison Ave. for future expansion.
