IMMIGRATION EXAM INCLUDES: Physical Exam, Blood Work, Urine Analysis, and TB Test. If needed, referral for x-ray and immunization clinics will be discussed at the time of your first exam.

EXAM REQUIREMENTS: Minimum of 2 appointments required. (first with nurse and second with doctor)

Nurse appointments (Available Monday-Friday) Call for appointment.

Doctor appointments (Available every other Friday afternoon) Call for appointment.

Please bring to your first appointment: photo id, proof of vaccinations, and USCIS Form 693 page one completed.
