Midtown Health Center Earns Six HRSA Quality Awards 

Midtown Health Center has been honored by the Health Resources and Services Administration (HRSA) with six Community Health Quality Recognition (CHQR) awards, based on 2025 clinical performance data — marking the second consecutive year the organization has earned all six recognitions. CHQR awards are announced annually by HRSA based on the prior calendar year’s Uniform Data System (UDS) performance data. 

The annual CHQR awards recognize health centers nationwide that demonstrate excellence in clinical quality, health outcomes, health equity, and the effective use of technology to improve patient care. Midtown’s repeat performance places it among a small group of Nebraska health centers to achieve this distinction two years running. 

Midtown Health Center received: 

  • Health Center Quality Leader (Gold) – Awarded to only three Nebraska health centers, this recognition places Midtown among the top 10% of community health centers nationwide for overall clinical quality performance. 
  • Advancing Health Information Technology (HIT) for Quality – Recognizes health centers that make effective use of health IT, including electronic health records, telehealth, patient engagement tools, clinical information exchange, and social risk factor data collection, to strengthen patient care. 
  • Behavioral Health – Midtown is one of only three Nebraska health centers recognized for expanding access to high-quality behavioral health services, including mental health and substance use disorder care. 
  • Preventive Health – Recognizes consistent performance across preventive care measures such as screenings, immunizations, and early detection services. 
  • Diabetes Health – Just three Nebraska health centers earned this honor, recognizing Midtown’s strong outcomes in diabetes prevention and management. 
  • Heart Health – Recognizes excellence in helping patients manage and improve their cardiovascular health.
