When you’re ready to apply and enroll in a health plan, HealthCare.gov is the place for that, too!

No matter where you live, you may be able to buy insurance from private health plans that cover a comprehensive set of benefits, including doctor visits, hospital stays, preventive care, and prescriptions. And plans in the Marketplace must treat you fairly—they can’t deny you coverage because of a pre-‐existing condition. ABOUT OUR SLIDING FEE SCALE

We are committed to serving the insured and uninsured. A sliding fee scale is available to make treatment affordable for those who choose to apply. Fees are calculated based on household size and documented annual income level.

Funding

This program is supported by the Health Resources and Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) as part of an award totaling $2,067,208. Additional federal funding for 2023-2024 brings the total to an estimated $2,389,212 with 75 percent financed with non-governmental sources. The contents are those of this brochure are those of Midtown Health Center and do not necessarily represent the official view of, nor an endorsement, by HRSA, HHS, the U.S. Government, or other funding agencies. Midtown also receives grant funding from: Madison County, Nebraska Department of Health and Human Services, Norfolk Area United Way, Women’s Fund of Omaha. This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health related claims, including medical malpractice claims, for itself and its covered individuals.
